How to import form into OSCAR.
1. From Schedule window in the top Menu go to "Admin" (at the top right corner)
2. Choose Forms/eForms and then Manage eForms
3. Under eForm Library choose Import (you have 3 choices: Upload Import Download).
   Click on "Browse" button and locate downloaded zip file with the form. Click "Import" button.

Old version of OSCAR
1. From Schedule window in the top Menu go to "Admin" (at the top right corner)
2. In the new window under "eForms" header choose "Upload eForm"
3. New window opens, choose "Import eForm" (second top left tab on top). Click on "Browse" button and locate zip file with the form. Click "Import" button.

For Apple users.
How do I stop automatically unzipping downloaded zip files in Mac Safari?
If you use Safari - just download the .zip file and leave it for you to decide what to do:

    Open Safari 
    Click Preferences
    Under the General tab, uncheck the option Open “safe” files after downloading
